Physical Therapist

Physical Therapist - Home Health Make a meaningful impact where patients feel most comfortable — at home. We're looking for a compassionate and motivated Physical Therapist to join our growing home health team and help patients regain mobility, restore independence, and improve their overall quality of life. What You'll Do As a Physical Therapist, you will provide individualized rehabilitative care to patients recovering from illness, injury, or surgery in the comfort of their homes.

Responsibilities include:

Perform and document comprehensive initial evaluations and assessments Develop and implement personalized treatment plans to improve mobility, strength, balance, and function Monitor patient progress and modify treatment plans to maximize outcomes Educate patients and families on exercises, safety, and ongoing care needs Maintain accurate and timely clinical documentation Collaborate with physicians, caregivers, and interdisciplinary healthcare teams Follow infection control and safety protocols at all times Support the agency's mission of delivering compassionate, high-quality patient care Qualifications Graduate of an accredited Physical Therapy program Current state Physical Therapist license CPR Certification required Valid driver's license and auto insurance Home health experience preferred HomeCare HomeBase experience preferred Physical Requirements Ability to stand, walk, bend, kneel, squat, and climb for extended periods Ability to lift, carry, push, or pull up to 100 lbs.
