Job Description

Position Summary:
The Physical Therapist (PT) provides skilled physical therapy services to patients to improve physical function, mobility, and quality of life while promoting safety and independence in activities of daily living (ADLs). The PT evaluates, develops, and implements individualized treatment plans tailored to the unique needs of each patient. The PT collaborates with an interdisciplinary team to ensure the highest level of care.
Key Responsibilities:
Evaluation and Treatment Planning:
Conduct comprehensive physical therapy evaluations for patients referred for services. Develop and implement individualized plans of care (POC) based on evaluation results, patient goals, and clinical judgment. Reassess progress and modify treatment plans as needed to achieve optimal outcomes.
Therapeutic Interventions:
Provide evidence-based therapeutic interventions to address functional limitations, reduce pain, and enhance mobility. Train patients in the use of assistive devices and adaptive equipment as needed. Educate patients and caregivers on home exercise programs to promote long-term improvement.
Documentation and Compliance:
Document evaluations, treatment sessions, progress notes, and discharge summaries in accordance with facility and Medicare/insurance guidelines. Maintain compliance with all state and federal regulations, including Medicare and Medicaid billing requirements. Ensure documentation supports medical necessity and justifies skilled therapy services.
Collaboration and Communication:
Work closely with interdisciplinary team members, including physicians, nurses, occupational therapists, speech therapists, and social workers, to develop comprehensive care plans. Participate in care plan meetings and family conferences as needed to discuss patient progress and treatment recommendations. Communicate effectively with patients, families, and caregivers to foster understanding and engagement in the rehabilitation process.
Supervision and Oversight:
Supervise Physical Therapy Assistants (PTAs) in accordance with state licensure laws, ensuring adherence to treatment plans and quality of care. Provide mentorship and training to PTAs and other rehabilitation staff as needed.
Professional Development:
Stay current with best practices, emerging research, and advancements in physical therapy through continuing education. Maintain active state licensure and certifications required to practice.
Qualifications:
Education:
Graduate of an accredited Physical Therapy program (DPT preferred, but MPT or BSPT acceptable based on licensure requirements).
Licensure:
Current and valid state licensure as a Physical Therapist.
Experience:
Prior experience in physical therapy is preferred, but not required.
Work Environment:
Work is performed in a variety of settings. The role may involve standing, walking, lifting, and transferring patients. Requires the ability to safely operate therapy equipment and assist patients with mobility.
Work Schedule:
