Job Description

Job Title:
Temp - PT - Long Term Care (LTC) (Days)
Position Overview:
We are seeking a licensed Physical Therapist to provide long-term care services in a facility setting. The successful candidate will evaluate, develop, and implement patient treatment plans, administer therapeutic procedures, monitor patient progress, and collaborate with healthcare team members to ensure optimal patient outcomes. This role includes supervising support staff and adhering to all regulatory and professional standards.
Key Responsibilities:
Screen, evaluate, and rehabilitate patients with physical functioning disorders and wounds. Develop and implement individualized plans of care based on assessments. Conduct procedures such as manual therapy, ambulation, therapeutic exercises, modalities, and wound care. Monitor and document patient progress, adjusting treatment plans as necessary. Provide patient and caregiver education, support, and counseling. Communicate effectively with the healthcare team regarding patient status and care plans. Complete comprehensive discharge summaries in compliance with regulations. Maintain accurate documentation within electronic health records. Record treatment minutes and participate in meetings including care conferences and reviews. Obtain necessary DME and supplies and supervise support staff including PTAs and aides. Comply with all professional ethical standards, including APTA guidelines and state practice acts.
