Weeks Position Overview:
Seeking an experienced Home Health Physical Therapist to provide one-on-one skilled therapy services to patients throughout the Medford, Oregon area. Candidates must have prior skilled home health experience and be proficient with OASIS documentation. Required Qualifications Minimum 1 Year of Skilled Home Health Physical Therapy Experience OASIS Experience Required BCLS Certification Valid Physical Therapist License Own Reliable Transportation Required PrimaryCOVID-19

Not Accepted Key Responsibilities Perform comprehensive physical therapy evaluations Develop and implement individualized treatment plans Provide skilled therapy interventions in patients' homes Complete OASIS assessments and documentation accurately Educate patients and caregivers on treatment plans and home exercise programs Coordinate care with physicians, nurses, and interdisciplinary team members Maintain timely and accurate clinical documentation inEPIC/OASIS
