• 08/22/2026) Schedule = Primarily outpatient (approximately 80% outpatient / 20% inpatient) Weekends = Occasional Saturday coverage (typically 2 Saturdays during the 13‑week contract) On‑Call = None Patient Ratio = Varies by caseload EMR = Epic Description of job: SnapCare is seeking a motivated and experienced Physical Therapist to support the Outpatient Rehab
• STARS Baker City Department at Saint Alphonsus Medical Center in Baker City, OR.

This role is primarily outpatient‑focused, with a small percentage of inpatient responsibilities. The Physical Therapist will provide evidence‑based rehabilitation services while collaborating with an interdisciplinary healthcare team to support patient recovery, functional improvement, and overall well‑being. This opportunity offers a consistent schedule with limited weekend expectations.

Qualifications:

Minimum 1 year of recent Physical Therapist experience Current BLS certification Active Oregon Physical Therapist license Experience working in outpatient rehabilitation settings Familiarity with Epic EMR preferred Strong clinical assessment, communication, and documentation skills

Responsibilities:

Evaluate patients and develop individualized treatment plans Provide therapeutic exercises, manual therapy, and functional training Educate patients and families on treatment plans and home exercise programs Monitor patient progress and modify plans of care as needed Document treatments and outcomes accurately in Epic EMR Collaborate with physicians and interdisciplinary care teams Maintain compliance with facility policies and professional standards
