Outpatient Physical Therapist (PT)
Location:
Portland, Oregon Job Overview We are seeking an experienced Outpatient Physical Therapist to join an innovative adult care program focused on helping older adults maintain independence and continue living safely within their communities. This role involves working closely with a geriatric population through comprehensive rehabilitation services in an outpatient setting. Schedule 4 x 8-Hour Shifts No Weekends No On-Call Requirements Active Physical Therapist License (pending license considered) BCLS Certification required Minimum 1 year of experience working with seniors required Experience with geriatric patients highly preferred Epic experience required First-time travelers not accepted Responsibilities Evaluate and treat adult and geriatric patients with a variety of physical and functional impairments. Develop individualized treatment plans to improve mobility, strength, balance, endurance, and overall function. Provide patient and caregiver education to support safety and independence. Collaborate with interdisciplinary healthcare teams to optimize patient outcomes. Document evaluations, treatment plans, progress notes, and discharge summaries in Epic.
