Home Health Physical Therapist (PT)
Location:
Seaside, OR Duration - 13 Weeks Job Overview We are seeking an experienced Home Health Physical Therapist (PT) to provide skilled therapy services to patients in their homes. This role is ideal for a therapist who enjoys working independently, managing a caseload, and helping patients improve mobility, function, and quality of life in a home-based setting. Schedule 8-Hour Day Shifts No Weekends No On-Call Requirements Active Physical Therapist License (pending license accepted) BCLS Certification required Minimum 1 year of Skilled Home Health experience required OASIS experience required First-time travelers accepted Reliable personal transportation required Responsibilities Perform comprehensive patient evaluations in the home setting. Develop and implement individualized treatment plans. Provide therapeutic interventions to improve strength, mobility, balance, and functional independence. Complete OASIS assessments and required home health documentation. Educate patients and caregivers on treatment plans, safety, and home exercise programs. Coordinate care with physicians, nurses, and interdisciplinary healthcare teams. Maintain accurate and timely documentation using the agency's electronic medical record system.
