8-Hour Day Shift Position Overview Upstream Rehabilitation is seeking an experienced Physical Therapist (PT) to provide exceptional outpatient orthopedic rehabilitation services at its Eugene, Oregon clinic. The Physical Therapist will evaluate patients, develop individualized treatment plans, implement evidence-based interventions, and monitor patient progress to help individuals restore mobility, reduce pain, and improve overall function. Key Responsibilities Perform comprehensive physical therapy evaluations and assessments. Develop and implement individualized treatment plans based on patient needs and goals. Provide evidence-based therapeutic interventions, including therapeutic exercises, manual therapy, gait training, and functional mobility training. Monitor patient progress and modify treatment plans as appropriate. Educate patients and caregivers on treatment plans, home exercise programs, injury prevention, and wellness strategies. Maintain accurate and timely clinical documentation in accordance with facility and regulatory standards. Collaborate with physicians, therapy staff, and other healthcare professionals to ensure coordinated patient care. Ensure compliance with all facility policies, state regulations, and professional practice standards. Position Details 13-week contract assignment 8-hour day shifts 36 hours guaranteed per week with the expectation of working up to 40 hours Both local and travel candidates are welcome Local candidate radius: 50 miles Coverage assignment No floating to other clinics No on-call responsibilities Patient Population Outpatient Orthopedic patients Required Qualifications Current Physical Therapist (PT) license in Oregon or eligibility to obtain licensure prior to assignment. Graduate of an accredited Physical Therapy program. Outpatient orthopedic experience is preferred. Prior travel experience is required. First-time travelers will not be considered. New graduates will not be considered. Strong clinical assessment, communication, and patient education skills. Overtime & Compensation Daily overtime after 24 hours (based on applicable multiplier). Weekly overtime after 40 hours (based on applicable multiplier). Overtime must be approved by the Director before any overtime hours are worked.
