Weeks Position Overview:
Seeking an experienced Home Health Physical Therapist to provide one-on-one patient care in the home setting. The ideal candidate will have skilled home health experience, strong assessment skills, and proficiency with OASIS documentation.
Required Qualifications:
Active Physical Therapist License (Pending License Accepted) BCLS Certification Minimum 1
Year of Skilled Home Health Experience OASIS Documentation Experience Required Valid Driver's License and Reliable Transportation Required Clinical Skills:
Home Health Physical Therapy OASIS Assessment and Documentation Patient Evaluation and Reassessment Treatment Planning and Implementation Mobility and Gait Training Strengthening and Balance Programs Fall Prevention Education Functional Independence Training Patient and Caregiver Education Key Responsibilities:
Perform comprehensive physical therapy evaluations in patients' homes Develop and implement individualized treatment plans Monitor patient progress and adjust treatment as needed Complete accurate OASIS and clinical documentation Educate patients and caregivers on treatment goals and home exercise programs Coordinate care with physicians, nurses, and interdisciplinary healthcare teams Maintain compliance with home health regulations and documentation standards For more details reach at or Call / Text at 732 791 4807
