Home Health Physical Therapist (PT) |
Travel Contract Location:

Hours Per Week Responsibilities:
Provide physical therapy evaluations, strength restoration, and mobility training in patients' homes. Manage a daily home health caseload and complete scheduled patient visits. Perform initial patient assessments, functional evaluations, and safety assessments. Develop and implement individualized treatment plans. Complete OASIS documentation accurately and efficiently. Document patient progress, outcomes, and care coordination activities. Utilize Epic systems for point-of-care charting and communication. Collaborate with interdisciplinary healthcare teams to support patient care goals. Educate patients and caregivers regarding therapy plans and home safety. Manage daily travel and scheduling between patient visits. Maintain compliance with home health regulations and agency standards.
Requirements:
Minimum 1 year of recent skilled Home Health experience. Active Oregon Physical Therapist (PT) License (pending Oregon license accepted). OASIS documentation experience required. Current BCLS (AHA) certification. Valid driver's license and clean driving record. Reliable personal transportation required.
