1 Description American Traveler is seeking a Home Health Physical Therapist with at least 1 year of skilled home health experience for a 13-week contract covering West Portland, OR and surrounding areas. Details Home health setting serving West Portland, OR and surrounding areas 1:1 patient ratio with up to 6 patient visits per day; 30 productivity points is standard 8-hour day shifts, Monday-Friday (4x10 and 3x12 schedules cannot be accommodated) No weekend or on-call requirements Travelers may be expected to float between facilities within the state Charting is completed via Epic Remote Client (laptop) and Rover app (phone), with OASIS documentation Travel radius of up to approximately 60 miles per day, with up to 60-90 minutes of total drive time Requirements Minimum 1 year of skilled home health PT experience required Active Oregon PT license required (license required if applicable) Current BLS certification required Epic EMR experience required OASIS experience required Must have reliable personal transportation Primary

vaccination series plus one booster required At least 2 supervisory references covering 1 year of employment within the last 3 years in a comparable specialty or setting required for consideration SSN and DOB required for consideration Additional Information Open to first-time travelers Local candidates are accepted; candidates living within 50-75 miles of the facility are considered local and are eligible at the same rate Former permanent staff must wait at least 3 months before returning as a traveler, and may not be within 50 miles of the submitting facility Business casual attire required; scrubs are permitted in any color, but jeans are not allowed Scheduling is designed to group clients geographically to minimize drive and windshield time RTO requests should be discussed prior to submission; minimal RTO is preferred Floating between facilities within the state is expected; refusal to float may result in assignment termination
