Job Description

Looking for a change of scenery and a chance to help others. Provide skilled physical therapy services in a home health setting in Roseburg, OR. Perform patient evaluations and deliver therapeutic interventions according to physician orders. Supervise and evaluate physical therapy assistants and home health aides as assigned. Participate in patient-care documentation, discharge planning, patient and family education, and agency in-service activities. Travel within the agency service area and perform home visits. Reliable transportation, current driver's license, and agency-required liability insurance are required. Minimum Qualifications Graduate of an accredited physical therapy program (Bachelor's, Master's, or Doctoral degree) approved by relevant accrediting bodies. Current, unencumbered Physical Therapist license for the state of Oregon required prior to start. Current CPR certification required. Experience in home health or community-based physical therapy preferred. Familiarity with electronic documentation, e-mail, and basic computer navigation required. Must have reliable transportation and a valid driver's license for home visits. Ability to supervise and evaluate PTAs and home health aides as part of clinical duties. Two to three years of clinical physical therapy experience preferred. Preferred Qualifications Previous experience managing caseloads in home health settings preferred. Experience performing in-home functional assessments and developing home-based treatment plans preferred. Experience with state-specific home health regulations and documentation preferred.
Assignment Details Shift:
Evenings, 5 shifts per week, 8 hours per shift. May require travel within the agency service area to perform home visits. and you will be contacted by a recruiter who will give you more information.
