Overview Join our dynamic healthcare team as a Home Health Physical Therapist and make a meaningful difference in patients' lives by delivering expert physiotherapy care in the comfort of their homes. In this vital role, you will assess patient needs, develop personalized care plans, and implement effective treatment strategies to promote recovery, mobility, and independence. Your expertise will help bridge the gap between hospital care and community living, ensuring seamless discharge planning and ongoing support for diverse patient populations. This position offers an engaging opportunity to apply your physiology knowledge, clinical skills, and passion for patient care in a rewarding home health environment. Part-time with full-time potential. Responsibilities Conduct comprehensive assessments of patients' physical condition, including mobility, strength, balance, and functional abilities. Develop and implement individualized care plans tailored to each patient's unique needs, goals, and medical history. Deliver evidence-based physiotherapy interventions such as therapeutic exercises, trigger point therapy, and orthopedic techniques to improve patient outcomes. Coordinate discharge planning with healthcare teams to ensure smooth transitions from inpatient or outpatient settings to home care. Educate patients and their families on injury prevention, activity modifications, and self-management strategies to promote long-term health. Maintain accurate documentation of treatment sessions, progress notes, and care plans using electronic medical record (EMR) systems. Collaborate with multidisciplinary teams including physicians, nurses, and social workers to optimize patient care across home and community settings. Qualifications Valid licensure as a Physical Therapist with relevant state or national regulatory bodies. Minimum of two years of experience; home health experience is not required. Strong knowledge of physiology, anatomy, medical terminology, and physiotherapy techniques such as trigger point therapy and orthopedics. Experience working with patients recovering from acute care or hospital stays. Ability to utilize EMR systems efficiently for documentation and communication purposes. Excellent discharge planning skills combined with a solid understanding of care plans tailored to individual patient needs.
Pay:
$48.00 - $52.00 per hourBenefits:
