Job Summary:

We are seeking a dedicated andpassionate Travel Physical Therapist (Home Health) to join one of our client's care teams. In this role, you will provide patient-centered, in-home therapy services focused on improving mobility, relieving pain, and restoring functional independence. You will work closely with physicians and interdisciplinary team members to develop and implement individualized plans of care. This position offers a rewarding opportunity to make a direct impact on patients' lives in a home health setting.

Primary Responsibilities:

• Evaluate patient functional status and physical therapy needs in accordance with CMS guidelines
• Develop and implement individualized treatment plans in collaboration with physicians
• Provide evidence-based therapy to relieve pain, restore function, and maintain independence
• Monitor and document patient progress, reporting changes to the care team
• Educate patients, families, and caregivers on treatment plans, goals, and home exercises
• Travel within assigned service areas to provide in-home patient care
• Complete timely documentation including evaluations, progress notes, and discharge summaries
• Participate in care team meetings and interdisciplinary collaboration
• Supervise Physical Therapist Assistants (PTAs) and home health aides as permitted
• Ensurepliance with all state regulations andanizational policies

Qualifications:

• Active Physical Therapist license
• Current CPR certification
• Valid driver's license, auto insurance, and reliable transportation

Pay Details:
