Position Summary:
The PRN Physical Therapist (PT) provides diagnostic, evaluative, and therapeutic physical therapy services to individuals with Intellectual and Developmental Disabilities (IDD). This role focuses on improving functional mobility, strength, balance, posture, safety, and overall physical independence through evidence-based interventions delivered in home and community-based settings.
Position Scope:
This PRN position supports rehabilitation services for individuals receiving IDD services. The Physical Therapist works independently while collaborating closely with interdisciplinary teams to ensure coordinated, person-centered care. Services are provided on an as-needed basis depending on participant and program needs
Key Responsibilities:
The Physical Therapist conducts evaluations, identifies mobility and safety needs, and develops and delivers individualized, evidence-based treatment plans to improve functional mobility, strength, balance, and gait. This role provides hands-on therapy in home and community settings, monitors progress, adjusts treatment as needed, collaborates with interdisciplinary teams, educates caregivers on safe mobility techniques, recommends adaptive equipment, and maintains timely, accurate documentation in accordance with licensure and regulatory requirements.

State Physical Therapy Practice Act and licensure regulations Applicable healthcare and IDD service regulations Organizational policies and procedures Ethical standards of physical therapy practice
Confidentiality:
The Physical Therapist must maintain strict confidentiality of all protected health information (PHI) and personal data in accordance with HIPAA and organizational confidentiality policies.
Documentation Requirements:
Complete evaluations, treatment plans, progress notes, and discharge summaries accurately and timely Maintain records in accordance with payer, regulatory, and organizational requirements Document services and outcomes using approved systems and formats
Certifications:
Current and valid Physical Therapist license in the applicable state CPR certification (preferred or required per organizational policy)
Education:
Doctorate or Master's Degree in Physical Therapy from an accredited program
Experience:
Experience working with individuals with Intellectual and Developmental Disabilities (IDD) preferred. Experience in home-based or community settings preferred.
Technical Skills:
Functional mobility and gait assessment Balance and postural control interventions Adaptive and assistive equipment recommendations Clinical documentation systems Evidence-based physical therapy techniques
Languages:
English required Bilingual skills are a plus depending on population served
Driver License:
Valid driver's license required. Acceptable motor vehicle record required.
Travel Requirement:
Travel required to home and community-based service locations. Scheduling flexibility required due to PRN status.
