Deliver individualized physical therapy sessions for assigned clients Perform initial evaluations, reassessments, and treatment planning Complete daily documentation and progress notes Submit monthly clinical reports Collaborate with clinical and residential staff on client‑specific needs Participate in Clinical Team and family meetings Contribute to the development and adjustment of predicted outcomes and care plans What We're Looking For Active Pennsylvania Physical Therapist license (required) Master's or Doctor of Physical Therapy from an APTA‑accredited program Strong communication and interpersonal skills Ability to work independently and as part of an interdisciplinary team Proficiency with EHR systems and Medicare documentation Preferred (not required) : 1+ year outpatient PT experience Experience with neurological or brain injury populations About Success Rehabilitation, Inc.
