Physical Therapist (PT) - Part‑Time Success Rehabilitation, Inc. Provide one‑on‑one physical therapy sessions for assigned clients Perform initial evaluations and reassessments Document treatment sessions and complete progress notes Submit monthly clinical reports Collaborate with the interdisciplinary clinical and residential teams Participate in clinical team and family meetings as needed Help develop and adjust individualized treatment goals and predicted outcomes What We're Looking For Active Pennsylvania Physical Therapist license (required) Master's or Doctor of Physical Therapy from an APTA‑accredited program Strong communication and interpersonal skills Ability to work independently and collaboratively Experience with Medicare documentation and EHRs Preferred (not required): 1+ year outpatient PT experience Experience with neurological or brain injury populations About Success Rehabilitation, Inc.
