Pay Per Visit (PPV) — rates based on visit type and home health experience About the Role A well-established home health and hospice organization is seeking a compassionate Physical Therapist to join its care team across multiple territories in Pennsylvania and Michigan. This is a full-time, community-based role for PTs who enjoy working independently, building relationships with patients in their homes, and being part of a collaborative clinical team that genuinely cares about outcomes.
