Pay Per Visit (PPV) — rates based on visit type and home health experience About the Role A well-established home health and hospice organization is seeking a compassionate Physical Therapist to join its care team across multiple territories in Pennsylvania and Michigan. This is a full-time, community-based role for PTs who enjoy working independently, building relationships with patients in their homes, and being part of a collaborative clinical team that genuinely cares about outcomes. Key Responsibilities Deliver skilled physical therapy services to patients in residential home settings Develop and implement individualized care plans in coordination with the broader care team Complete clinical documentation accurately and on time using EMR software Manage your own schedule and caseload within the assigned territory Travel locally to patient homes — approximately 20% of your day is driving Qualifications Bachelor's or Master's degree in Physical Therapy from an accredited program Active PT license in the state where you'll be working (PA or MI) Minimum 1 year of healthcare experience (new grads may be considered) Valid driver's license and reliable transportation Experience with EMR documentation preferred (HomeCare HomeBase a plus) Ability to self-manage time, schedule, and meet charting deadlines Additional Information Flexible Monday-Friday schedule with 1 weekend every 4-6 weeks DailyPay — access your earnings when you need them Full benefits package including health, dental, and vision Company-provided tablet and smartphone Up to 90% tuition discount for you and your family Comprehensive onboarding with clinical educators and mentors Hiring Process Qualified candidates will complete a brief phone pre-screen before being connected with the hiring team.
