Job Overview Join our dynamic healthcare team as a Physical Therapist specializing in Skilled Nursing Facilities (SNF) on a contract basis! In this vital role, you will deliver expert physiotherapy services to patients across inpatient and outpatient settings, including hospitals, clinics, and home health environments. Your expertise will help improve mobility, reduce pain, and promote recovery for diverse patient populations, from geriatrics and orthopedics to pediatrics. This position offers an exciting opportunity to apply your physiology knowledge and care planning skills in a fast-paced, rewarding setting where every patient's progress matters. Duties Conduct comprehensive assessments of patients' physical conditions, utilizing your deep understanding of anatomy and physiology to develop personalized care plans. Implement evidence-based physiotherapy techniques, including trigger point therapy and orthopedic interventions, to optimize patient outcomes. Manage discharge planning processes by coordinating with multidisciplinary teams to ensure smooth transitions from hospital or inpatient settings to home or community care. Deliver targeted therapy sessions for diverse populations such as geriatrics, pediatrics, and those recovering from surgery or injury. Document patient progress meticulously within Electronic Medical Record (EMR) systems, ensuring accurate and timely updates of treatment plans and medical terminology. Collaborate with healthcare professionals across inpatient, outpatient, hospital, and home health environments to align treatment strategies with overall patient care goals. Educate patients and their families on physiotherapy exercises and injury prevention techniques to promote long-term wellness. Qualifications Valid licensure as a Physical Therapist with current registration in the relevant jurisdiction. Proven experience in acute care, hospital settings, or outpatient clinics with a strong understanding of medical terminology and anatomy. Demonstrated expertise in discharge planning, care plans development, and home & community care coordination. Familiarity with EMR systems for documentation and patient management. Experience working with diverse populations including geriatrics, pediatrics, orthopedics, and patients requiring trigger point therapy. Knowledge of physiotherapy techniques such as orthopedics treatment modalities and trigger point therapy. Strong communication skills to effectively collaborate with multidisciplinary teams across inpatient, outpatient, hospital, and home health environments. $2,250.00 - $2,400.00 per week
