Utilize Net Health and PointClickCare (PCC) Position Responsibilities Evaluate residents and develop individualized treatment plans Provide skilled physical therapy services in a SNF/LTC setting Collaborate with interdisciplinary team members to maximize resident outcomes Complete timely and accurate documentation using Net Health Educate residents, families, and caregivers on treatment plans and progress Maintain compliance with all regulatory and facility requirements Qualifications Graduate of an accredited Physical Therapy program Current PT license in the State of Pennsylvania (or eligible) Strong communication and interpersonal skills Passion for providing exceptional patient-centered care Experience in SNF/LTC preferred, but not required New graduates are welcome and encouraged to apply Benefits Include Medical, Dental, and Vision Insurance Paid Time Off 401(k) If you're looking for a rewarding opportunity with a team that values compassion, clinical excellence, and professional growth, we'd love to hear from you!
