Key Responsibilities MSW:

Conduct psychosocial assessments and develop care plans Provide short-term counseling and crisis intervention Coordinate community resources and referrals Educate patients/families on advance directives and coping strategies

PT:

Evaluate patients' physical condition, strength, and mobility Develop and implement individualized treatment plans Restore function, reduce pain, and prevent disability Educate patients and caregivers on home exercise programs Requirements for Both Roles Current, active California license (MSW or PT) Master's degree in Social Work (for MSW) / Doctorate or Master's in PT (for PT) Minimum 1 year of home health experience preferred Valid driver's license, reliable vehicle, and auto insurance Current CPR/BLS certification Ability to travel to patient homes across Northern California How to Apply Please submit your resume through Indeed.
