We are looking for professionals who are committed to clinical excellence, compassionate care, and improving patient outcomes through personalized rehabilitation services.

Position Responsibilities:

Conduct patient evaluations and establish individualized treatment plans Deliver evidence-based physical therapy services in accordance with physician orders Assist patients in improving mobility, strength, balance, and overall functional independence Maintain accurate and compliant patient records and progress notes Communicate effectively with patients, caregivers, and care teams Participate in care coordination and interdisciplinary collaboration

Required Qualifications:

Current Florida Physical Therapist License 2-5 years of relevant clinical experience preferred Home health experience strongly preferred Knowledge of Medicare guidelines and documentation standards Strong organizational and time-management skills Excellent verbal and written communication skills Ability to work independently in a hybrid environment
