Trinity Home Health is seeking an experienced Per Diem Physical Therapist to serve patients throughout the San Luis Obispo North County area . We are looking for a compassionate, independent clinician with prior home health experience who enjoys providing high-quality care in the comfort of patients' homes. Position Summary The Physical Therapist will primarily be responsible for completing: Start of Care evaluations (SOCs) Recertifications Discharges Initial evaluations and reassessments This role requires strong clinical skills, excellent time management, and the ability to work independently while collaborating with an interdisciplinary team. Responsibilities Perform comprehensive physical therapy evaluations in the home setting. Complete Start of Care assessments, recertifications, and discharge visits. Develop individualized treatment plans to maximize patient function and independence. Provide patient and caregiver education. Collaborate with physicians, nurses, and other healthcare professionals to coordinate care. Complete documentation accurately and timely within the EMR. Maintain compliance with Medicare and home health regulations.
