Job Description

Physical Therapist (PT) - St George Utah Full-Time |
Home Health Schedule:
Monday-Friday, 8am-5pm Position Summary CNS is seeking a compassionate Full-Time Physical Therapist to support Home Health patients in the greater St George Utah area. This role offers meaningful 1:1 care, greater autonomy, and the opportunity to work with patients needing care in home.
Compensation & Benefits Per Visit:
$65.00 to $95.00 - per visit, indeed will not let us enter a per visit rate.
Mileage:
$0.57/mile reimbursement

Better work-life balance than facility-based shifts and patient ratio 1:1 patient care with strong clinical support Autonomy with fewer interruptions Meaningful relationships built in the home environment Key Responsibilities Provide skilled physical therapy services to Home Health patients in their homes Conduct comprehensive patient evaluations and develop individualized treatment plans Implement therapeutic exercises, manual therapy, gait training, and functional mobility interventions Educate patients, families, and caregivers on home exercise programs and safety modifications Coordinate care with physicians, nurses, occupational therapists, social workers, and interdisciplinary team members Monitor patient progress and adjust treatment plans as needed Maintain accurate and timely documentation per CMS, state, and CNS standards Adhere to safety, infection control, and HIPAA guidelines Qualifications Current Utah PT license in good standing Graduate of an accredited physical therapy program (DPT preferred) CPR/BLS certification required Home health experience preferred; pediatric, geriatric, neurological, or orthopedic experience a plus Strong clinical assessment, critical thinking, and problem-solving skills Excellent communication and interpersonal skills Ability to work independently and manage time effectively with field-based caseload Valid driver's license, reliable transportation, and current auto insurance Physical Requirements Ability to lift, transfer, and assist patients with mobility (up to 50 lbs frequently) Ability to drive throughout the St George area and surrounding communities Ability to sit, stand, walk, bend, kneel, and squat for extended periods Ability to carry therapy equipment and supplies to patient homes This job summary outlines the general nature and level of responsibilities and is not an exhaustive list of all duties or skills required.
