About Mass General Brigham Home Care Mass General Brigham Home Care provides post-acute, medically necessary skilled care in the home to help homebound patients recover from illness, surgery, or injury and manage exacerbations of chronic disease. What Makes Home Care Physical Therapy Different? As a Physical Therapist on our Lighthouse team, you will:

• Deliver 1:1 physical therapy care in patients' homes
• Evaluate patients and develop personalized treatment plans
• Focus on mobility, strength, fall prevention, and functional independence
• Assess home environments to promote safety and independence
• Educate patients and caregivers on recovery strategies and long-term mobility

Provide physical therapy services to patients in their place of residence who have physical impairments, functional limitations or disabilities resulting from injury, disease or other causes. Patients may range in age from birth to over 100 years. Collaborate with patient, family, physician and other allied health professionals to identify patient needs and coordinate services to meet those needs. Key Activities Develop a plan of care that is patient and family focused and institutes standardized interventions for goal-oriented outcomes Daily documentation is accurate and timely Application of standard treatment procedures Teaching is provided to patients and caregivers Qualifications Qualifications and Experience One year of experience in acute care, rehab or homecare as a Physical Therapist Registered with the Board of Registration in Massachusetts.
