This is a wonderful opportunity for a brand-new Physical Therapist or a Physical Therapist who is interested in learning the home health field. Home care experience is a plus, but it is not required . We are willing to train and support the right candidate who is dependable, eager to learn, and passionate about providing quality patient care in the home setting. Compensation Excellent starting pay rate from $48 and up . Salary is negotiable based on experience. Employment Type Full-time and part-time positions available. Position Overview The Cedars has an immediate opening for a Home Health Physical Therapist . This position reports directly to the Therapy Team Coordinator and Clinical Director . The Home Health Physical Therapist is responsible for assessing patients in their homes, developing and implementing individualized plans of care, providing therapy services, and helping patients improve mobility, strength, safety, and overall independence. Training and support will be provided for candidates who are new to home health care. Required Skills and Qualifications Doctorate or Master's Degree from an accredited Physical Therapy Program Current unrestricted Physical Therapy license in the State of Rhode Island Brand-new Physical Therapists are welcome to apply Home health experience is a plus, but not required Clinical experience from an inpatient hospital, skilled nursing facility, or outpatient setting is preferred Current CPR certification Strong organizational and communication skills Good interpersonal skills Reliable transportation Active auto insurance Clean driving record required Weekend availability is a plus Must be dependable, professional, and comfortable traveling to patients' homes Duties and Responsibilities Assess home health patients to identify physical therapy needs Develop and implement individualized plans of care Provide skilled physical therapy services in the patient's home Support patients with mobility, strength, balance, safety, and functional independence Participate in proper discharge planning to ensure quality and continuity of care Provide clinical direction to Physical Therapist Assistants when available Ensure consistent, high-quality patient care Follow insurance requirements and documentation standards Demonstrate professionalism, commitment, and competency Promote the agency's mission and values Follow generally accepted standards of physical therapy practice Communicate effectively with patients, families, physicians, and the clinical team
