Home Care Physical Therapist

Job description Pinnacle Home Care, LLC. is currently seeking an experienced Full Time Physical Therapist (PT) for the North & Metro areas in RI. We provide one-on-one skilled physical therapy to patients living in their homes. PT develops and implements a plan of treatment in conjunction with the physician and the patient. PT collaborates with other interdisciplinary team members in case managing the care and treatment necessary to improve the patient's health condition.

Responsibilities:

Evaluate and assess patients' physical conditions and develop appropriate treatment plans Implement therapeutic exercises and techniques to improve patients' mobility, strength, and range of motion Perform diagnostic tests and measurements, such as the mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments. Develop and implement appropriate individualized care plans. Assess and revise the Physical Therapy care plan every 30 days. Educate and instruct patients on activities necessary to promote their health, safety, and independent living. Make home visits to patients. Accurately document observations, interventions, and evaluations utilizing a touch pad tablet that is provided to you by the company.

Qualifications:

Graduate of an accredited school of physical therapy with minimum of 1 year experience. Current license to practice as a physical therapist in RI. Valid CPR license Valid drivers license and proof of insurance coverage. Ability and willingness to work in a team environment. Ability to adjust schedule availability to meet the needs of the patients. Ability to travel between patient assignments within a 25-mile radius.
