What You'll Do:

As a Home Health Physical Therapist, you'll manage a caseload of patients from admission through discharge, providing comprehensive evaluations, developing individualized treatment plans, and delivering therapeutic interventions. You'll collaborate with a supportive interdisciplinary team to achieve the best possible outcomes for each patient.

Your Day-to-Day Might Include:

• Performing in-home physical therapy evaluations and mobility assessments
• Developing and adjusting personalized treatment plans based on patient goals
• Providing direct therapeutic interventions to improve function and independence
• Educating patients and caregivers on safe practices, exercises, and mobility tools
• Coordinating care with nurses, occupational and speech therapists, and case managers
• Documenting treatment and progress accurately and timely in compliance with policies
• Participating in interdisciplinary meetings and contributing to quality improvement

What You'll Need:

• Education and qualifications required for RI Physical Therapy licensure (required)
• Bachelor's Degree in Physical Therapy or related field (strongly preferred)
• Valid Rhode Island PT license and CPR certification (required)
• At least one year of clinical experience (home health or acute care preferred)
• Valid driver's license and automobile liability insurance
