Responsibilities:

Provide in-home physical therapy services to patients across the Tri-County area. Evaluate, plan, and implement personalized treatment plans. Collaborate with interdisciplinary team members to ensure optimal patient outcomes. Educate patients and families on care plans and safe practices.

Requirements:

Current Physical Therapist (PT) license in South Carolina Prior experience in home health (preferred but not required) Reliable transportation and valid driver's license Strong communication and clinical skills Compassionate, self-motivated, and team-oriented Proficient and timely with EMR point of care documentation If you're ready to work in an environment where your contributions truly matter—and where people know your name —we'd love to welcome, you to our team.
