Job Description

A hospital in Cheraw, SC is seeking a Physical Therapist to join its outpatient rehabilitation department. This role focuses on providing direct therapy services that support patient recovery, mobility improvement, and long-term functional independence in an outpatient clinical setting. Position Overview The Physical Therapist will assess patients, develop individualized treatment plans, and deliver evidence-based rehabilitation services. Care is provided in collaboration with other clinical team members to ensure coordinated, patient-centered outcomes across the continuum of outpatient rehabilitation services. The role requires strong clinical reasoning, effective communication, and the ability to manage a varied caseload while working independently within established clinical guidelines. Key Responsibilities Conduct outpatient physical therapy evaluations and assessments Develop and implement individualized treatment plans based on patient needs Deliver therapeutic interventions to improve strength, mobility, and function Monitor and document patient progress and treatment outcomes Collaborate with physicians and clinical staff to coordinate care plans Educate patients on home exercise programs and recovery strategies Maintain compliance with clinical protocols and safety standards Adjust treatment plans based on patient response and progress Support continuity of care through effective documentation and communication Qualifications Graduate of an accredited Physical Therapy program required Active and unrestricted Physical Therapy license in South Carolina required Current CPR certification required Previous outpatient or rehabilitation experience preferred Strong clinical judgment and communication skills required Benefits Medical, dental, and vision insurance Paid time off and holiday benefits Retirement savings program Continuing education and professional development support Stable outpatient clinical environment Opportunities for professional growth within rehabilitation services
