Category Homehealth - PT Job Type Contract Offering a vital opportunity for an experienced Physical Therapist to provide home health services to adult patients, this contract role is designed for professionals with a background in home-based chronic disease management. The position supports the delivery of tailored care to adults in the region, focusing on therapeutic interventions, assessment, and patient education in a home health setting.
Schedule:
8-hour day shifts (0800 - 1700) 40 hours per week No on-call or call-back requirements No weekend or regular floating; work holidays as scheduled
Qualifications & Experience:
Active Physical Therapist license (state licensure required) Minimum of a BA degree in a related field One year of experience, preferably in home health (first-time travelers considered if seasoned in the specialty) Valid driver's license and proof of insurance BLS certification Must reside at least 50 miles from the designated home unit
Required Skills:
Recent charting experience with Homecare Homebase (mandatory) and OASIS-E Strong proficiency in assessment, IV management, pleurx, wound vac, wound care, and educating adults with chronic illnesses Experience with geriatric/adult patients and chronic disease management documentation
Responsibilities:
Conduct approximately six visits (total points) per day, providing evidence-based therapy to adult patients Manage care plans for chronic conditions, perform assessments, maintain accurate, timely records, and educate patients for improved disease self-management Utilize department-issued technology (Samsung Note 9, EMR system Homecare Homebase) Collaborate with departmental leadership and participate in orientation programs to ensure high standards of care
