  • 50/hour | PRN | Flexible Scheduling About Us Fountain Inn Post Acute is a 66-bed skilled nursing facility located just 20 minutes from downtown Greenville, South Carolina.

  • 50/hour Flexible PRN scheduling Reliable hours available PRN opportunities within the largest network in South Carolina Potential full-time opportunities Supportive, collaborative rehab team Position Summary As a Physical Therapist (PT), you will evaluate and treat residents to improve mobility, strength, and functional independence.
You will collaborate with physicians, families, and interdisciplinary team members to provide high-quality, individualized care. Key Responsibilities Perform patient evaluations and develop individualized treatment plans Provide therapy to improve strength, mobility, balance, and overall function Monitor patient progress and adjust treatment plans as needed Maintain accurate and timely clinical documentation Communicate with physicians, residents, families, and care team members Participate in care planning and discharge planning Conduct staff education and in-service training Qualifications Degree in Physical Therapy (Bachelor's required; Master's or Doctorate preferred) Active, unencumbered Physical Therapist license in South Carolina (required) Must maintain all continuing education and remain in good standing with the State Board Experience in a long-term care or rehab setting preferred Familiarity with PointClickCare (PCC) and Casamba preferred Strong clinical, communication, and organizational skills Why Join Fountain Inn Post Acute?
