  • 50/hour | PRN | Flexible Scheduling About Us Chandler Creek Post Acute is a 133-bed skilled nursing facility located in Greer, South Carolina.

  • 50/hour Flexible PRN scheduling based on your availability Reliable PRN hours available Opportunities across the largest post-acute network in South Carolina Supportive and collaborative rehab team environment Position Summary As a Physical Therapist (PT), you will evaluate and treat residents to improve mobility, function, and overall quality of life.
You will play a key role in helping patients regain independence and achieve personalized therapy goals. Key Responsibilities Perform patient evaluations and develop individualized treatment plans Deliver therapeutic interventions to improve strength, balance, and mobility Monitor patient progress and adjust treatment plans as needed Maintain accurate and timely documentation Collaborate with interdisciplinary teams to support patient outcomes Educate residents and families on therapy techniques and recovery goals Qualifications Bachelor's degree required; advanced degree preferred Active, unencumbered Physical Therapist license in South Carolina Experience in a long-term care or rehab setting preferred Familiarity with PointClickCare (PCC) and Casamba preferred Strong communication and clinical skills Ability to work independently and as part of a team Why Join Chandler Creek Post Acute?
