Job Description

Category Homehealth - PT Job Type Contract Deliver exceptional care and support to adult patients as a Physical Therapist in a home health setting. This contract position offers you the opportunity to focus on chronic disease management and promote patient well-being, all while benefitting from a structured 8-hour day schedule.
Schedule & Hours:
8-hour day shifts 40 hours per week
Shift:
8:00 AM - 5:00 PM No weekend rotation No on-call or call-back requirements Holiday work as scheduled
Desired Experience and Qualifications:
Minimum of 1 year of clinical Physical Therapy experience At least some prior home health experience required (first-time travelers considered if seasoned in the field) Active PT license in South Carolina Bachelor's degree or higher in Physical Therapy BLS certification Valid driver's license and proof of insurance Proficient with charting in Homecare Homebase (required) Experienced with OASIS-E assessments Demonstrated skills in chronic disease management, wound care, IV/pleurx/wound vac care, and adult/geriatric assessments
Key Responsibilities:
Perform home health visits with an average of 6 points per day Deliver evidence-based rehabilitation and therapy for adult patients Complete timely and accurate documentation using Homecare Homebase on Samsung Note 9 devices Educate patients and families on disease processes, management, and post-therapy care Coordinate with department leadership and interdisciplinary teams for optimal outcomes Adhere to safety and compliance protocols, including maintaining proper records of home health visits
