Job Summary Join our dynamic healthcare team as a Physical Therapist specializing in Skilled Nursing Facilities (SNF). In this vital role, you will deliver expert physiotherapy services to patients across various age groups and medical conditions, promoting recovery, independence, and improved quality of life. Your expertise will help develop personalized care plans, facilitate discharge planning, and coordinate with multidisciplinary teams to ensure comprehensive patient care. This position offers an energetic environment where your skills in acute care, outpatient clinics, home health, and inpatient settings will make a meaningful difference in patients' lives. Responsibilities Assess patients' physical conditions through detailed evaluations, including physiology and anatomy knowledge, to create tailored care plans. Implement therapeutic interventions such as trigger point therapy, orthopedics techniques, and geriatrics-focused exercises to enhance mobility and reduce pain. Coordinate discharge planning by collaborating with healthcare teams to ensure seamless transition from hospital or inpatient settings to home or community care. Deliver patient care across diverse environments including hospital wards, outpatient clinics, home health visits, and skilled nursing facilities. Maintain accurate documentation using electronic medical record (EMR) systems, recording treatment progress and updating care plans regularly. Support patients with specialized needs such as pediatrics or complex medical conditions by applying appropriate physiotherapy techniques. Educate patients and their families on injury prevention, physiotherapy exercises, and health management strategies to foster long-term wellness. Skills Strong knowledge of physiology and anatomy essential for effective assessment and treatment planning. Experience with discharge planning processes and managing care transitions across inpatient, outpatient, and home health settings. Proficiency in EMR systems for documentation and patient record management. Familiarity with medical terminology relevant to physiotherapy and multidisciplinary healthcare teams. Hands-on experience in acute care environments such as hospitals or inpatient units. Skilled in a variety of physiotherapy techniques including trigger point therapy, orthopedics, geriatrics, and pediatrics. Ability to work independently while collaborating effectively with physicians, nurses, social workers, and other healthcare professionals.
