Job Description

Overview Join our dynamic healthcare team as a dedicated Physical Therapist, where your expertise will transform patient recovery journeys across the home health spectrum. In this vital role, you will assess, plan, and implement personalized physiotherapy programs that promote mobility, reduce pain, and enhance overall quality of life. Your proactive approach to discharge planning and care coordination will ensure seamless transitions for patients across the continuum of care. Responsibilities Conduct comprehensive assessments of patients' physical conditions, including mobility, strength, and functional abilities. Develop and implement tailored care plans utilizing evidence-based physiotherapy techniques such as trigger point therapy and orthopedic interventions. Collaborate with multidisciplinary teams to coordinate discharge planning and ensure continuity of care in inpatient, outpatient, or home & community settings. Provide expert patient care by educating individuals and their families on treatment protocols, exercises, and injury prevention strategies. Manage documentation efficiently using Electronic Medical Record (EMR) systems to record progress notes, treatment plans, and discharge summaries. Apply specialized skills in pediatrics, geriatrics, orthopedics, and sports medicine to address diverse patient needs across age groups. Monitor patient progress regularly and adjust treatment strategies to optimize recovery outcomes while adhering to medical terminology standards. Qualifications Bachelor's or Master's degree in Physiotherapy or an equivalent healthcare qualification. Valid licensure or registration as a licensed Physical Therapist in the relevant jurisdiction. Proven experience in acute care hospital environments with familiarity in inpatient and outpatient settings. Strong knowledge of physiology, anatomy, medical terminology, and therapeutic techniques including trigger point therapy. Experience working with diverse populations such as pediatrics, geriatrics, orthopedics, and those requiring home & community care. Proficiency with EMR systems for accurate documentation and patient record management. Pay:
$58.60 - $67.14 per hour
