1 Description American Traveler is seeking a Physical Therapist with home health experience for a 13-week contract providing adult home-based therapy services in York County, SC. Details Home health setting serving York and Lancaster counties in South Carolina, with possible occasional coverage in Union or Mecklenburg County Patient population is adults only with common diagnoses including chronic diseases, wound care, IV therapy, pleurx, and wound vac Caseload of approximately 6 points (visits) per day Day shift schedule, 8 hours/day (0800-1700), 40 hours/week No on-call, call-back, or weekend rotation requirements Holidays will be worked as scheduled
EMR:
Homecare Homebase (required); OASIS-E assessment tool experience required Requirements Minimum 1 year of home health physical therapy experience required; first-time travelers considered if seasoned in home health Active PT state license required (license required if applicable) BLS certification required Bachelor's degree (BA) in Physical Therapy or related field required Homecare Homebase EMR experience required — no exceptions OASIS-E assessment tool experience required Experience with IV therapy, pleurx, wound vac, and wound care required Experience with geriatric/adult populations, chronic disease management, and maintenance notes required Valid driver's license and proof of auto insurance required Candidate's permanent address must be located 50+ miles from the home unit; local candidates are not accepted Minimum 1 professional reference dated within the past year from a Supervisor, Manager, Director, or Charge RN Last 4 digits of SSN and full date of birth required for consideration Additional Information Daily responsibilities include patient assessment, disease teaching, wound care, IV and pleurx management, and maintenance note documentation Support provided by departmental leadership Samsung Note 9 phones are used in the field 5-day orientation (NEO) provided Grey scrubs required for therapy staff Candidates who have worked a permanent position at an AHSE facility must have been separated for at least 1 year before being considered
