This full-time opportunity is ideal for a clinician who is passionate about improving mobility, restoring function, and enhancing quality of life for residents in a skilled nursing and long-term care environment. The Physical Therapist will evaluate residents, develop individualized treatment plans, and provide evidence-based therapeutic interventions in a supportive, patient-centered setting. Key Responsibilities Evaluate residents' physical abilities, mobility limitations, and rehabilitation needs Develop and implement individualized physical therapy treatment plans Provide therapeutic exercises and interventions to improve strength, balance, mobility, and independence Assist residents with gait training, transfers, and functional mobility activities Monitor patient progress and modify treatment plans as appropriate Maintain accurate and timely clinical documentation in accordance with facility standards Collaborate with physicians, nursing staff, and interdisciplinary care teams to support patient outcomes Educate residents and family members regarding therapy goals and home exercise programs Promote a positive, patient-focused rehabilitation environment Qualifications Graduate of an accredited Physical Therapy program Active South Dakota Physical Therapist license or eligibility for licensure Strong clinical assessment and treatment planning skills Excellent communication and teamwork abilities Skilled nursing, rehabilitation, or long-term care experience preferred New graduates are encouraged to apply Preferred Skills Experience working in skilled nursing or post-acute rehabilitation settings Ability to manage multiple patient caseloads efficiently Commitment to compassionate, resident-centered care Strong documentation and organizational skills For more details contact at or Call / Text at 516-862-1169.
