We hire therapists who share our vision and dedication, working diligently to restore function, prevent decline, and support patients in performing meaningful daily tasks. ✅ About the Facility Ahava Therapy Services is seeking a Full-Time PT to join our in-house rehab team at Henry County Health and Rehabilitation Center, a 136-bed skilled nursing facility providing short-term rehabilitation, long-term care, and outpatient services. Conduct and document thorough functional assessments using standardized procedures. Develop and implement individualized treatment plans with achievable goals. Comply with evaluation, treatment, and documentation guidelines; maintain timely, accurate records. Instruct and supervise therapy assistants as needed. Enter treatment and progress notes for each session. Revise treatment plans as needed throughout the course of care. Document treatment in clear, concise discharge summaries. Orient residents to services and treatment procedures appropriate to their needs. Collaborate with nursing staff, residents, and families to maximize functional abilities. Educate residents and caregivers on the continuation of home exercise and therapy. Assist with referrals for additional services as needed and maintain knowledge of community resources. Participate in rehabilitation team conferences and other facility meetings. Access continuing education opportunities to maintain clinical competency. Participate in quality improvement initiatives. Follow regulatory, professional, and facility standards. Perform other duties as assigned to provide quality care. Understand payer reimbursement and allocate therapy resources efficiently.
