Home Health Physical Therapist

Responsibilities Assess patients' physical conditions through comprehensive evaluations, including mobility, strength, and functional capacity. Develop and implement individualized care plans tailored to each patient's needs, goals, and medical history. Provide hands-on physiotherapy interventions such as trigger point therapy, orthopedics, geriatrics, pediatrics, and outpatient rehabilitation techniques. Coordinate discharge planning with healthcare teams to ensure seamless transitions from inpatient or hospital settings to home care. Educate patients and caregivers on exercises, injury prevention, and health management strategies to promote ongoing recovery. Document patient progress accurately using electronic medical record (EMR) systems and maintain detailed treatment records. Collaborate with multidisciplinary teams on care plans that include outpatient clinics, inpatient facilities, or hospital-based programs. Requirements Valid licensure as a Physical Therapist with current registration in the applicable region. Proven experience in home health or community care environments with knowledge of home & community care best practices. Strong understanding of physiology, anatomy, medical terminology, and physiotherapy techniques such as trigger point therapy. Experience managing diverse patient populations including geriatrics, pediatrics, orthopedics, and those recovering from acute care or hospital stays. Familiarity with EMR systems for documentation and patient record management. Ability to perform discharge planning effectively while coordinating with hospital teams or outpatient clinics. Excellent communication skills to educate patients and collaborate with healthcare providers effectively. $38.86 - $46.80 per hour
