Position Overview As a Physical Therapist (PT), you'll play a key role in evaluating residents, developing care plans, and delivering physical therapy interventions that improve mobility, relieve pain, and restore function. This position offers a rewarding opportunity to enhance the lives of residents through evidence-based, individualized therapy. Key Responsibilities Conduct assessments to determine mobility deficits, pain levels, and rehabilitation goals. Design and implement individualized treatment plans to address each resident's physical challenges. Deliver skilled physical therapy services in accordance with physician orders. Collaborate with interdisciplinary team members to support optimal patient care. Educate patients, families, and caregivers on home exercise programs and safe mobility practices. Maintain accurate, timely, and thorough documentation per facility and regulatory standards. Participate in care planning meetings and provide recommendations for continued care. Ensure compliance with all federal, state, and local regulations. Qualifications Active Texas Physical Therapist license In good standing with all licensing and regulatory bodies Experience in Skilled Nursing Facilities (SNF) or long-term care preferred Familiarity with Net Health documentation system is a plus Excellent communication and clinical reasoning skills
