PRN Physical Therapist (PT) - In-House SNF Conroe Health Care Center - Conroe, TX Conroe Health Care Center is excited to offer an opportunity for a PRN Physical Therapist (PT) to join our in-house rehabilitation team in Conroe, TX . This role is ideal for a clinician who is passionate about delivering high-quality, resident-centered care in a collaborative skilled nursing environment. At Conroe Health Care Center , we focus on individualized therapy programs that support functional improvement, safety, and quality of life. Our in-house model allows for strong interdisciplinary collaboration and consistent clinical standards. Compensation starting at $55/hr and will increased based on level of experience. Position Overview As a Physical Therapist , you will evaluate residents, develop personalized treatment plans, and provide skilled therapy services aimed at improving mobility, strength, balance, and functional independence. Key Responsibilities Evaluate residents' physical conditions and functional limitations Develop and implement individualized treatment plans using evidence-based practices Provide therapeutic interventions to restore mobility and reduce pain Monitor resident progress and adjust treatment plans as needed Educate residents, families, and caregivers on exercises and mobility strategies Complete accurate and timely documentation per facility and regulatory standards Collaborate with the interdisciplinary team to ensure optimal resident outcomes Maintain compliance with all safety and regulatory requirements Qualifications Active Florida Physical Therapist (PT) license In good standing with all licensing boards and regulatory agencies SNF or long-term care experience preferred Familiarity with Net Health documentation system a plus Strong communication and clinical skills Why Join Conroe Health Care Center?
