Our team includes skilled nurses and contracted health professionals committed to delivering quality palliative and end-of-life care to adults in the Greater Houston area. Summary We are seeking a Physical Therapist to join our team at White Orchid Hospice. In this role, you will provide specialized physical therapy services to patients in their homes, ensuring they receive the highest level of care while maintaining their comfort and dignity during this critical time. Responsibilities Conduct comprehensive assessments of patients' physical therapy needs in a home setting. Develop individualized treatment plans based on patient assessments and goals. Implement therapeutic exercises and interventions to improve patients' mobility and quality of life. Educate patients and their families on physical therapy techniques and home exercises. Collaborate with interdisciplinary teams to coordinate patient care effectively. Document patient progress and adjust treatment plans as necessary. Requirements Licensed Physical Therapist in the state of Texas. Experience in geriatrics or hospice care preferred. Strong knowledge of physiology and medical terminology. Ability to work independently and manage time effectively in a home health environment. Excellent communication skills for interacting with patients, families, and healthcare teams.
