Responsibilities:
- Conduct thorough evaluations of patients' physical conditions and develop personalized treatment plans.
- Work with home health patients to improve mobility, alleviate pain, and enhance overall well-being.
- Collaborate with interdisciplinary healthcare teams to ensure comprehensive patient care.

Qualifications:
- Valid Physical Therapist license in Texas.
- Minimum 1 year of experience in physical therapy and mild exposure to home health
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Proficient in using mobile apps for organizational purposes.

Geriatrics Home Health Independent Contractor Physical Therapist (PT)Essential Job Functions/Responsibilities:
Evaluate the patient's functional status and physical therapy needs. Assists the physician in the development of the therapy plan of care. Provides services within the scope of practice as defined by the state board of physical therapy, in accordance with the plan of care, and in coordination with other members of the health care team. Treats patients to relieve pain, restore function, and / or maintain maximum functioning using acceptable standards of practice. Instructs and informs participating members of the health care team, the patient, and the family/caregivers regarding the plan of care, functional limitations, and progress toward goals. Visits patients according to Plan of Care, completes progress notes timely. Provides supervision of PTAs Participates in discharge planning for patient.Position Qualifications:
Must be currently licensed to practice as a Physical Therapist in theSTATE OF TEXAS
Must possess and maintain valid CPR certification. Valid driver's license, auto insurance and reliable transportation- Ability to work independently and as part of a team.
