Opportunities available in the heart of Kaufman We are seeking a Kaufman with a passion for service excellence to join our Family and Pediatric Healthcare team in the Kaufman Area. The Physical Therapist (PT) evaluates, plans, and provides therapeutic interventions to patients with injuries, illnesses, or movement disorders to improve mobility, reduce pain, restore function, and prevent disability. The PT collaborates with physicians and interdisciplinary healthcare teams to develop individualized treatment plans that support patient recovery and overall wellness.

) Here's a sneak peek at what you'll do : Evaluate patients' physical conditions, mobility limitations, strength, balance, and functional abilities. Develop individualized treatment plans based on patient assessments and goals. Provide therapeutic exercises, manual therapy, stretching, gait training, and other physical therapy interventions. Educate patients and caregivers on treatment plans, home exercise programs, injury prevention, and recovery strategies. Monitor patient progress and modify treatment plans as needed to achieve optimal outcomes. Maintain accurate and timely documentation in the electronic medical record (EMR) system. Collaborate with physicians, nurses, occupational therapists, and other healthcare professionals to coordinate patient care. Assist patients in improving mobility, pain management, strength, flexibility, and independence in daily activities. Ensure compliance with safety standards, infection control procedures, and regulatory guidelines. Participate in quality improvement initiatives and patient care planning meetings. What you need to succeed Doctor of Physical Therapy (DPT) degree from an accredited program required. Current and unrestricted Physical Therapist license in the applicable state. Current CPR/BLS certification required. Strong knowledge of physical therapy techniques, rehabilitation protocols, and patient care standards. Experience in primary care, family medicine, community health, or outpatient settings preferred. Excellent patient engagement and communication skills Experience in community health centers and/or non-profits is preferred Experience serving multicultural and multilingual patient populations Experience with eClinicalWorks or similar MS Office Suite skills (Outlook, Teams, Word, PowerPoint and Excel) Have a servant heart with the desire to make a positive impact What We Offer At HHM Health, our mission starts with caring for people and that includes you.
