This position offers the opportunity to work independently while making a meaningful impact on patients in the comfort of their homes. We are looking for a clinician who is patient-focused, dependable, and passionate about improving mobility, safety, and overall quality of life.

Responsibilities:

Evaluate and develop individualized treatment plans for home health patients Provide therapeutic exercises, gait training, balance training, and mobility education Assess patient progress and modify treatment plans as needed Educate patients and families on home exercise programs and safety Collaborate with physicians, nurses, and interdisciplinary team members Complete timely and accurate documentation

Qualifications:

Active Texas Physical Therapist license required Home Health experience preferred, but not required Strong clinical and communication skills Ability to work independently and manage schedule effectively Current CPR certification Reliable transportation and valid driver's license
