Job Description

Physical Therapist (PT) - Per Diem (PRN) Shinnery Oak Community 711 W Broadway Street, Denver City, TX 79323 Join Our In-House Rehab Team Shinnery Oak Community is seeking a skilled and compassionate Physical Therapist (PT) for a per diem (PRN) opportunity within our in-house subacute rehabilitation program. This role offers flexibility while allowing you to make a meaningful impact on the lives of our residents. Job Summary The Physical Therapist (PT) is responsible for evaluating, planning, and implementing individualized treatment programs for residents in a subacute and long-term care setting. The PT works collaboratively with an interdisciplinary team to help patients improve mobility, restore function, and achieve their highest level of independence. Responsibilities Evaluate residents and develop individualized treatment plans based on physician orders Provide skilled physical therapy services in accordance with established plans of care Assess patient progress and modify treatment plans as needed Document evaluations, treatments, and patient progress in compliance with company policies and regulatory standards Collaborate with physicians, nursing staff, and rehabilitation team members Educate patients, families, and caregivers on therapeutic exercises and care plans Ensure all services are delivered in accordance with state and federal regulations Participate in care plan meetings and interdisciplinary team discussions as needed Qualifications and Skills Current Physical Therapist (PT) license in the state of Texas required SNF or subacute/long-term care experience preferred Strong clinical assessment and treatment planning skills Knowledge of current physical therapy practices and regulatory guidelines Experience with electronic documentation systems (Net Health preferred) Excellent communication and interpersonal skills Ability to work independently and as part of a team This is an excellent opportunity for a Physical Therapist seeking a flexible PRN schedule in a supportive, team-oriented environment.
