00 per hour , with increases based on experience and qualifications. We are seeking a compassionate and dedicated Physical Therapist (PT) to join our rehabilitation team on a PRN basis. We are looking for a clinician who is committed to helping patients regain independence, improve mobility, and achieve meaningful functional outcomes. Our therapy team provides high-quality, person-centered care in a collaborative environment. We focus on individualized treatment plans to support residents in reaching their highest level of function and improving quality of life.
Responsibilities:
Evaluate residents and develop individualized treatment plans based on clinical findings and physician orders Administer skilled physical therapy treatments to improve mobility, restore function, reduce pain, and promote healing Monitor patient progress and modify treatment plans as clinically appropriate Collaborate with physicians, nursing staff, families, and interdisciplinary team members to support optimal patient outcomes Educate residents, families, and caregivers on treatment plans, exercises, and safety techniques Maintain accurate and timely documentation in accordance with facility and regulatory requirements Participate in care planning meetings and interdisciplinary team discussions as needed
Qualifications:
Active Texas Physical Therapist license Graduate of an accredited Physical Therapy program SNF or Long-Term Care experience preferred, but not required Experience with Net Health documentation system is a plus Strong communication, clinical reasoning, and patient care skills
