Deliver targeted physiotherapy interventions such as trigger point therapy, orthopedics treatments, and geriatrics-focused exercises to promote healing and mobility. Collaborate with multidisciplinary teams to coordinate discharge planning, rehabilitation goals, and ongoing care strategies for diverse patient populations including pediatrics and elderly patients. Monitor patient progress regularly, adjusting treatment plans as necessary to optimize outcomes in inpatient, outpatient, home health, or hospital settings. Maintain accurate documentation of patient care activities within electronic medical record (EMR) systems while adhering to medical terminology standards. Educate patients and caregivers on exercises, injury prevention techniques, and home & community care practices to support long-term wellness. Skills Strong knowledge of anatomy and physiology essential for effective physiotherapy interventions. Experience with discharge planning and developing comprehensive care plans tailored to individual needs. Proficiency in EMR systems for documentation and patient record management. Hands-on experience in acute care, inpatient hospital environments, outpatient clinics, home health services, or community care programs. Skilled in various physiotherapy techniques including trigger point therapy and orthopedics treatments. Ability to communicate complex medical terminology clearly to patients and team members. Familiarity with care for diverse populations such as pediatrics and geriatrics, emphasizing patient-centered approaches.
