$55-$60/hour Sabine Heights is hiring a Physical Therapist to support a therapy program focused on mobility, safety, and functional outcomes across both short-term rehabilitation and long-term care. In this role, you will work closely with the interdisciplinary team to evaluate residents, guide treatment planning, and support mobility and function based on each resident's individual goals, whether focused on recovery, preserving independence, or enhancing quality of life. Physical Therapist:

Evaluating residents and guiding recovery following surgery, hospitalization, or functional decline Improving strength, balance, and gait to support safe, independent movement Identifying fall risk and implementing strategies that improve safety Supporting residents working toward discharge, while also helping long-term residents maintain independence and mobility Contributing to mobility and quality of life for residents at every stage of care What the work feels like here: A mix of short-term rehab and long-term care with varied clinical needs A therapy program that is integrated into daily resident care Collaboration with nursing and the interdisciplinary team A setting where clinical judgment and consistency matter What you'll be doing: Completing evaluations and developing individualized plans of care Treating residents with a focus on functional mobility, balance, and strength Supervising and collaborating with PTA support Monitoring progress and adjusting care based on clinical presentation Communicating with nursing and the care team on resident status Maintaining timely and accurate documentation What we're looking for: Active Texas PT license Skilled nursing or post-acute experience preferred Strong clinical judgment and confidence in decision-making Someone who values both independence and collaboration Learn more about us: www.sabineheights.com We are proud to be an EOE.
